How do we add patients to the Mobile Hub as walk ups if they do not have a drivers license or insurance information?

  • If a patient does not have a drivers license, you will have the opportunity to manually update the demographic information. These walk-up patients, as referenced above, only qualify for flu shots.

For walk-up patients, we recommend having a few items available:

  • If you need a Vaccine Information Sheet for any vaccine, they are available in your portal by visiting Resources > Forms > VIS Forms.
  • If you'd like to have a patient sign a consent form prior to administration, you can see our blank consent form here: http://help.vaxcare.com/en/articles/2792819-blank-consent-form
  • Note: Consent forms are not required on behalf of VaxCare & you will not need to send or submit these to us. These are only available for your convenience.
